Apparently, the answer, or at least part of the answer, is  picking up dealmakers like dealmakers pick up Hermes ties. Evercore Partners Inc. Chief Executive Officer Ralph Schlosstein said when he sees a banker with a proven track record, his instinct is to bring the dealmaker on board even if there isn’t a job opening. “Great bankers will figure out a way to make money,” Schlosstein said Tuesday during a conference hosted by Goldman Sachs Group Inc. “I once read a really great quote from Siegmund Warburg, and he was asked, how did he have so many great bankers at S.G. Warburg. And he said, ‘It’s very simple. I look at a great banker the way I look at a nice tie. If I see it, I buy it whether I need it or not.Ӊ€™ Another part of the answer rests in the fact that Evercore, like a couple other mid-sized Wall Street firms like Lazard and PJT Partners (formerly Blackstone's banking unit), has a business model that can cushion the blow of low profits during bear markets. “We have a benefit that you at Goldman Sachs don’t have, and that’s when things get a little tougher, we have a restructuring business,” Schlosstein said. Restructuring work generally picks up during economic slumps when there might be fewer banking opportunities tied to mergers and acquisitions. “One of the things that we can say to high-talent bankers is, ‘there’s more that you can do here in a down cycle.Ӊ€™ An interesting aside, which might shed some light on Evercore's  culture (which always receives  rave reviews from Evercore bankers who take Vault's annual banking survey), is Schlosstein's atypical career path to Wall Street CEO. He did his undergraduate work at the small liberal arts and sciences school Denison University in Ohio, and received his master's in public policy from the University of Pittsburgh. After that, for many years, he worked in several high profile government positions, including deputy to the assistant secretary of the U.S. Treasury Department and associate director of The White House Domestic Policy Staff under Jimmy Carter. In the 1980s is when Schlosstein began working in finance. He worked as an MD in investment banking with Lehman Brothers, then went on to found investment management behemoth BlackRock, where he served as president for two decades before leaving to start an alternative asset management firm. In 2009, he was named Evercore's CEO, taking over the reins from another man who first served his country before moving on to finance: Evercore Founder Roger Altman, who once held the position of Deputy Treasury Secretary under Bill Clinton. In any case, if nothing else, Schlosstein's jagged journey to the head of one of the most prestigious investment banks in the country, if not the world, should serve as a reminder that there's no straight path to the corner office with the best views. Paperwork. Dropping reimbursement rates. More paperwork. Increased patient loads of increasing complexity. Patients who are homeless, without transportation or without money for prescriptions. And yes more paperwork. The diagnosis? Physician burnout. Given that theres pressure on the production end and pressure on the complexity side, its not surprising that physicians experience burnout, says John Combes, MD, president and COO of the American Hospital Associations Center for Healthcare Governance. Survey after survey shows that doctor burnout is growing for GPs and specialists alike. The solution? The right medical staffing that can help doctors focus on what they love doing most treating patients. Start With Teamwork In reality, you cant hire your way out of burnout. You have to be smart about it, says Combes. The issue is how to reorganize care delivery into a much more team-based model, he says, where each professional is working to the full extent of their training. That might mean that, rather than focusing on 2,000 patients, a doctor is part of a team thats responsible for 20,000 patients. Those patients are cared for according to acuity, by the right staff. Heres how it works: A New Physician Role Physicians in this model would handle the most complex or acute cases, handing off maintenance, chronic care and minor illnesses to advanced practice nurse practitioners and handing paperwork to administrative staff. Within this new model physicians play another role as well as temp staff to help prevent physician burnout. Melissa Byington of the medical staffing agency CompHealthheads the companys locum tenens division, which supplies short-term physicians to departments when a physician leaves the team. In the last few years, Byington says more large medical centers are using locum tenens physicians to backfill staff during the hiring process. Finding physicians who are team players, who can easily adapt to new environments and who possess solid workplace communication skills are key characteristics for these roles. They need to be more flexible than most great communicators who fit in immediately, she says. Nurse Practitioners and Physician Assistants Top-performing nurse practitioners and physician assistants, can take some of the patient load off doctors shoulders. Byington says CompHealth has seen an increase in the number of requests for these providers to do exactly what Combes said: To take on more routine tasks so doctors can spend their time on more complex cases. Social Workers In many cases, the provider is tasked with taking care of the social needs of patients, says George Lowe, MD and medical director for two satellite clinics of Mercy Medical Center in Baltimore. Medical assistance at home, government assistance, home care all of these functions fall under the primary care doctors responsibility today. But in Lowes clinics, a social worker is on hand to address these needs, sometimes providing referrals, often doing the work themselves. Counselors/Therapists At the end of the day, most primary care doctors end up being front-line psychopharmacologists, says Lowea role that few physicians feel comfortable with. Lowes model provides licensed clinical social workers trained in therapy to do that work. It reduces stress on primary care doctors because thats another aspect of care that most primary care doctors are not well trained in and feel uncomfortable with, both in training and in time, Lowe says. They dont have 50 minutes to spend with patients on these issues. Medical Assistants At Lowes clinics, each of the 60 physicians and NPs are assigned a medical assistant who does the tasks that drain doctors energy: taking care of paperwork that theyre legally permitted to do, including Family and Medical Leave Act forms as well as forms for durable medical equipment, excepting scooters. Medical assistants also review health maintenance lists for patient follow up; they get medication and test authorizations from insurers. The point, says Lowe, is to reduce the hassle factor that drives doctors out of practice. Community Health Workers (CHWs) These health workers including health coaches, educators and navigators and health case managers connects the dots between physician orders and what the patient hears and does, says Dike Drummond, MD, an executive coach for physicians based in Washington state. CHWs often have little employee training, sometimes only as much as a community college certificate. At the CHW Certificate Program at City College of San Francisco, the oldest such program in the country, they train people to do fundamental care. CHWs learn to speak with patients in their own terms, translating medical terminology into language patients can understand and follow. Health case manager is the most recent role for CHWs, says Alma Avila, director of the San Francisco program. Graduates go to clinics and follow up with patients post-doctor visit. Theyre often the ones who go through directions, she says. If the patient doesnt read, they translate in a way the patient can understand. All of this makes a great deal of sense to Drummond. After all, physicians are expensive, and should spend their time doing what theyre trained to do. Heres the basic lament from all physicians: I just want to see patients, says Drummond. They just want to come in, listen to [the patients] chest, make the right diagnosis and offer effective treatment.

Today, well sum it all up by looking at the three major takeaways from our survey of 50 thriving senior executives: traverse with your edge, master your affiliation needs, and learn the difference  between provincial and cosmopolitan knowledge. Lesson No. 1: Traverse With Your Edge As we mentioned in No More Climbing the Corporate Ladder,  traverse is a skiing term that means to lead with the edge of your skis. Your ski edge gives you maneuverability. In career traversing, you lead with your skills edge. Your edge gives you maneuverability through different terrains. Here is an example of traversing from the career of James, one of the 50 executives we spoke with: After receiving his MBA from Columbia University, James went into banking. Various assignments at Mellon Bank and Bank of America eventually led to James being hired as President and CEO of a California bank. In 2009, Jamess bank was acquired and he was without employment. There was a consolidation of banks, so career continuation was impossible. James created a one-person consulting firm with an  initial focus on what he calls credit-dependent companies. Using his personal relationships with West Coast bank presidents, James was able to negotiate settlements so that both sides could have something of value. By 2013, the recession had  ended, and one of James clients came to him for consulting assistance. The consulting  opportunity led to an offer to become chief operating officer of the clients organization. Jamess assignment as COO was to double the size of the medical products distribution company and then sell the company to a national player. This assignment was completed within eighteen months. Once again, James opened his consulting practice. One of his clients was a nonprofit organization. This consulting assignment brought him exposure to new areas like fund raising and working with agencies in Washington, D.C. This assignment was completed after two years. The contacts James developed brought him to the notice of a board member of a nonprofit company in his town. James was offered the position of chief executive officer for a California human services organization with a budget of $265 million. As you can see, James has been a bank president, a distribution company COO, and a nonprofit CEO. Between these W-2 employment assignments, he worked as a consultant on a project-by-project basis.  It was his 1099 assignments that led him to the W-2 assignments. If you view Jamess career from a ladder-climbers perspective, he  appears to have had a hodgepodge career. But that is not how James sees  it: I have centered my professional life on one strong theme: I solve financial and organizational problems from the perspective of a banker. Had I identified myself as a banker, my goose would have been cooked as the banking industry continued its consolidation. Instead, I have worked with medical products, retail companies, construction companies, and a giftware company. It has been fun, a real learning experience. But my core identity remains the same. It never changes: I solve financial and organizational problems from the perspective of a banker. Ted is another executive we spoke with whose career offers a great example of traversing with your skills edge: Ted began his IT career working with a variety of large corporations. Five years later, he moved to a technology consulting firm. Teds success as a consultant in an assignment involving ocean cargo issues led to an opportunity to become chief information officer for a company in the ocean freight transportation industry. Five years later, he was once again consulting. His  consulting assignments helped him gain credibility in the financial services sector, and Ted is now CIO for a global financial services company. In commenting on his professional life, Ted describes his edge as a constant even while his  assignments changed continuously: My skills are coaching and developing people in technical environments. Internal or external, I use the same tools. I just apply those tools in different ways. Lesson No. 2:  Master Your Affiliation Needs Affiliation is the desire to be an integral part of something larger than yourself. That something could be as small as a team that will finish a project this month or as large as an institution with a  mission to  make the world a better place. In the W-2 phase of a career, moderate degrees of affiliation are helpful; you are part of a corporate team. When you are in the 1099 phase, however, you are not really part of a clients team. You are helping a team for a period of time, and then you leave. How do you manage your affiliation needs and your professional relationships when you are traversing between W-2 and 1099 assignments? The answer is to focus on professional associations to meet your affiliation needs. Professional associations are work-related reference groups outside  of any specific corporation. These reference groups can focus on function/profession (e.g., American Psychological Association, Financial Executives International, American Marketing Association); industry (American Bar Association, Massachusetts Biotech Council); or geography (Chicago Chamber of Commerce, Downtown Crossing Association of Boston). Successful careerists take their association memberships seriously  and view them as  ways to  meet their affiliation needs. It is important to understand how critical these professional associations are for your career. In the 21st century, associations will play  the same role that trade guilds played in the Middle Ages: They will be sources of stable affiliation in an unstable project-oriented world. The connections you make in professional associations  will form the core professional network you need to help you traverse between 1099 and W-2 roles. Simply joining associations is not enough. You need to become known within each  association. You need to be a committee member,  not just someone who sometimes shows up for the occasional cocktail party. That means you need a strategy for each association you join, and you need to limit your associations so you avoid spreading yourself too thin. Lesson No. 3: Learn the Difference Between Cosmopolitan and Provincial Knowledge In the W-2 phase of a career, leaders are often hired to manage the work of others. Moving up the corporate career ladder means leaving the  technical mastery of doing stuff to other people. For example, a hospital CEO may have management responsibility over a surgeon, but that doesnt  mean we know how to perform surgery. The U.S. secretary of defense may be responsible for the conduct of the military but lack the skill to take apart a rifle. The skills necessary to do stuff at a tactical level are referred to as provincial knowledge. On the other hand, strategic vision and emotional intelligence â€" which increase in value as one moves up the corporate ladder â€" are what we call cosmopolitan skills because they can be of value in any industry or organization. Lou Gerstner took over IBM without any skills as an electronics engineer or even an appropriate background in IBMs technological foundation. George Marshall moved from being a soldier to running the Department of Defense, then to being secretary of state, and then to being president of the American Red Cross.
